Leeds Intermediate Care Service is a Star
Leeds mental health intermediate care service was highly commended in the Leeds Health Star Awards, with its use of Just Checking to improve assessment and care planning. The awards, run by NHS Leeds, highlight excellence in innovation and improvement across the city.

Herefordshire wins Accolade in Health and Social Care Awards
A clever combination of technology and well-trained domiciliary staff earned Herefordshire County Council and PCT an accolade in 2009 NHS Health and Social Care Awards. Herefordshire's mental health intermediate care service, runner up in the West Midlands regional final, achieved:
- • 43% reduction in residential care admissions
- • 57% of service users still living at home 1 year later
- • 63% of those living at home have low level needs at 1 year, indicating they will continue to live in the community
Herefordshire makes use of the Just Checking activity monitoring system to assess and monitor rehabilitation people with dementia. A roving night team logs on to the real time Just Checking data via smart mobile phone, and visits only those who are up at night and needing assistance.
